  • In case of light to moderate rain would you prefer your pet to stay dry inside the room with no outdoor access, or have access to the private concrete patio, or go into the play yards?
  • Any existing medical problems or abnormalities we should know about? (pancreatitis, permanent limp, metal plates, tumors etc.)

  • Are there any other physical problems or disabilities which may affect your dog in daycare? What other information or issues do you wish to address, or feel you should inform us of?
    I have read, understand, and agree to the terms and conditions for bringing my pet to Thousand Hills Pet Resort. (This contract will be emailed to you when you sent your application; you must sign and return it before your pet will be admitted.)

  • Up to date shot records

    Before we can make a reservation, we need your up to date shot records. Please send your records via fax to (805) 715-0507. Once we receive this information we will review your application and shot records, then call to confirm your reservation. Thank you for bringing your pets to play and to stay!
